We accepts cash, Visa, Master Card, Discover, and American Express. We do not accept personal checks or Care Credit. State sales tax will be charged where applicable.
All initial consultation are free of charge and are directed by our Patient Coordinator and/or Physicians Assistant.
Eddie Armas, M.D. Anti Aging Clinic reserves the right to charge $75.00 for a requested Doctor Consultation or interview for any procedure. This would be available toward credit to any service if purchased on the date of the consultation. Non- refundable in the scenario of not being treated by us on the appointment date.
Gift Certificates (Available for purchase at the front desk)
Gift Certificates are available for specified services, packages, or dollar amounts beginning at $50 and are not redeemable if lost or stolen. “Donation” certificates are non-transferable, and expiration dates may not be changed or extended. If an appointment is made to redeem your gift certificate and it is not cancelled at least 48 hours in advance, your treatments and your gift certificate will be subject to be forfeited.
Coupons (Groupon/Guilt/Living Social/Other)
New customers only. Limited 1 per person, may buy 1 additional as a gift. Coupon must be brought into the Clinic order to proceed with treatment. No substitution of service, combination with any other offers, promotions, gift cards or discount allowed. No shows or same day cancellations will be deducted 100 % of the service scheduled. Coupons for single treatments expire six month after redemption and one year for multi-treatment coupons. No cash refunds. Services are not transferable.
Pricing & Refunds
Pricing and treatments are subject to change without notice. Package options are non-refundable and non-transferable because of such changes. Exchange or credit for other services is only available within 30 days of receipt. Transaction service fee of 15% may apply on exchanges and credits. No returns on gift certificates or packages gift certificates. Unfortunately and without exception, we cannot accept items on opened product or edible items. All refunds are for in house CREDIT ONLY to be used on future product purchases or treatments.

Package extensions are not granted for vacation, travel, work or other personal events EXCEPT for medical reasons. To be considered for an extension, please submit a letter accompanied by a doctor’s note directed to the Manager. All treatments expire one year from date of purchase.
We kindly ask that any changes and cancellations are made within 24 hours of your appointment. Same day cancellations will be subject to a fee of 50% of your schedules treatment (not exceeding $100).” No shows” will be charged/deducted the full amount of the original service booked.
